- Complimentary CT scan $0
- Fluoride (optional) $59
2. Established Patient Visits (each visit includes Cleaning, Periodic Exam, X-rays, and Fluoride):
- 2 visits per year $599
- 3 visits per year $699
- 4 visits per year $799
